doc: cpp manual nits 

gcc/:
	* doc/cpp.texi (C++ Named Operators): Fix markup for header
	file name.
	* doc/cppinternals.texi (Top): Wrap node in @ifnottex to avoid
	two extra empty pages in PDF output.
