documentation_hacking.xml: Remove obsolete link to DocBook Publishing Tools.

* doc/xml/manual/documentation_hacking.xml: Remove obsolete link
	to DocBook Publishing Tools.
	epubcheck has moved to GitHub.
	Update URL of the DocBook Element Reference; use that term as
	link description instead of "online".
