iommu/amd: Refactor IOMMU tlb invalidation code



build_inv_iommu_pages() and build_inv_iommu_pasid() pretty much
duplicates the code. Hence enhance build_inv_iommu_pages() to
invalidate guest pages as well. And remove build_inv_iommu_pasid().
